Self-immolation

Our client, a dependent adult with a long history of serious mental illness, was under the care of a licensed facility tasked with her supervision and safety. Despite known risks of self-harm and prior conservatorship for being a danger to herself, she was allowed to leave the facility unsupervised. Hours later, she died after setting herself on fire.
